Rehabilitation to Reintegration (R2R)
R2R is a support service for people’s journey, pre, during and post discharge from residential rehabilitation who are returning to South Lanarkshire.
Key Elements
Support Workers would introduce R2R when the person is being considered for residential rehabilitation and be referred from their Care Manager, the work will be focused on helping the person to prepare to enter a residential facility.
While in the residential facility the support worker will keep in contact, helping where required, this also applies to anyone unexpectedly leaving the facility or leaving the facility early or unplanned
As the person work towards leaving the rehabilitation facility, the support worker will work with the person to identify their areas of interest and needs to help them once leaving
Working with the person as they leave residential rehabilitation facilities and transition into their communities utilising community rehabilitation.
Creating supportive environments through a trust-based relationships, R2R will help build resilience and confidence which can help reduce potential for relapse.
The support workers can also help with reconnections with loved one’s, we know this can be difficult issue to navigate. R2R can also link family members to family support if desired.
R2R operates in two strands
1. Practical and Emotional Support
The support worker will help the person prepare to enter a residential facility, for example,
- Getting their house in order, organising their personal affairs/ responsibilities for the period they are in RR, e.g., housing, legal, bills, family responsibility, pets.
- Accessing and utilising wraparound services; motivational interviewing; introducing the peson to and accompanying them to recovery meetings
- Increase the persons knowledge and undertanding of residential facilities and programmes
On leaving residential the support worker may help with
- Facilitated access to recovery opportunities and activities including recovery meetings, mutual aid, social activities and conversation cafes.
- Facilitated access to appropriate statutory and voluntary sector services including, benefits, housing, GP’s, family support and counselling.
- Individualised personal development opportunities catering to the unique strengths, interests, and areas of improvement of the person
- Support the person to address whole support needs i.e., housing, benefits, family connections
- Accompany the person to recovery focused opportunities including the Beacons, other recovery communities and mutual aid
Personal development planning
- Together the support worker and person will identify areas of interest and need, enabling the support worker to have opportunities, activities, training options and recovery opportunities in place for when the person leave the facility.
- Support workers will help the person navigate the landscape they’re returning to on leaving a residential facility

How to access R2R
R2R accept self-referrals and referrals from the persons CAReS Case Manager and from partner agencies working with people in the process of accessing Residential Rehabilitation.
